    Equity of access to adult hospice inpatient care within north-west England.

    There is a growing debate about the question of equity of access to hospice and palliative care services. Even countries with relatively well developed palliative care systems are considered to have problems of access and inequity of provision. Despite these concerns, we still lack a relevant evidence base to serve as a guide to action. We present an analysis of access to adult hospice inpatient provision in the north-west region of England that employs Geographical Information Systems (GIS). Measures of the possible demand for, and supply of, hospice inpatient services are used to determine the potential accessibility of cancer patients, assessed at the level of small areas (electoral wards). Further, the use of deprivation scores permits an analysis of the equity of access to adult inpatient hospice care, leading to the identification of areas where additional service provision may be warranted. Our research is subject to a number of caveats�it is limited to inpatient hospice provision and does not include other kinds of inpatient and community-based palliative care services. Likewise, we recognise that not everyone with cancer will require palliative care and also that palliative care needs exist among those with nonmalignant conditions. Nevertheless, our methodology is one that can also be applied more generally

    Possible identifications of the 3.4 micrometer feature

    A feature at 3.4 micrometer was first detected in Comet Halley by the IKS spectrometer on board the Vega 1 probe; and subsequently from the ground. The feature has since been reported in Comet Wilson. The presence of the feature is of considerable interest for a number of reasons. First, it may represent the detection of a new parent molecule, and when combined with data from Giotto and Vega yield new information on cometary chemistry and the early solar system composition. Secondly, it may represent a link to the interstellar medium, the feature corresponds in wavelength and shape with an interstellar feature seen in absorption in a luminous star, towards the Galactic center known as GC-IRS7. The feature in turn is thought to be related with a growing family of unidentified infrared emission features seen in stellar objects, planetary nebulae, reflection nebulae, HII regions and extra galactic sources. These features occur at wavelengths 3.3, 3.4, 3.5, 6.2, 7.7, 8.6, and 11.25 micrometers. Further identification theory is given

    Two Tunnels to Inflation

    We investigate the formation via tunneling of inflating (false-vacuum) bubbles in a true-vacuum background, and the reverse process. Using effective potentials from the junction condition formalism, all true- and false-vacuum bubble solutions with positive interior and exterior cosmological constant, and arbitrary mass are catalogued. We find that tunneling through the same effective potential appears to describe two distinct processes: one in which the initial and final states are separated by a wormhole (the Farhi-Guth-Guven mechanism), and one in which they are either in the same hubble volume or separated by a cosmological horizon. In the zero-mass limit, the first process corresponds to the creation of an inhomogenous universe from nothing, while the second mechanism is equivalent to the nucleation of true- or false-vacuum Coleman-De Luccia bubbles. We compute the probabilities of both mechanisms in the WKB approximation using semi-classical Hamiltonian methods, and find that -- assuming both process are allowed -- neither mechanism dominates in all regimes.Comment: 16 PRD-style pages, 13 figures.     Hope as a Source of Resilience in Later Adulthood

    This research provided a preliminary investigation of how variations in trait and state hope are associated with positive adaptation to stress in later adulthood. Trait hope and neuroticism were measured by questionnaires and state hope, stress, and negative emotions were assessed daily for 45 days. Results from multilevel random coefficient modeling analyses suggested that daily hope provides protective benefits by keeping negative emotions low, while also contributing to adaptive recovery from stress. The dynamic linkages between daily hope, stress, and emotion were further moderated by individual differences in trait hope. Compared with those low in trait hope, high-hope individuals showed diminished stress reactivity and more effective emotional recovery

    Modelling the influence of the process inputs on the removal of surface contaminants from Ti-6Al-4V linear friction welds

    The linear friction welding (LFW) process is finding increasing interest from industry for the fabrication of near-net-shape, titanium alloy Ti–6Al–4V, aerospace components. Currently, the removal of surface contaminants, such as oxides and foreign particles, from the weld interface into the flash is not fully understood. To address this problem, two-dimensional (2D) computational models were developed using the finite element analysis (FEA) software DEFORM and validated with experiments. The key findings showed that the welds made with higher applied forces required less burn-off to completely remove the surface contaminants from the interface into the flash; the interface temperature increased as the applied force was decreased or the rubbing velocity increased; and the boundary temperature between the rapid flash formation and negligible material flow was approximately 970 °C. An understanding of these phenomena is of particular interest for the industrialisation of near-net-shape titanium alloy aerospace components.EPSRC, Boeing Company, Welding Institut

    Comparing the states of many quantum systems

    We investigate how to determine whether the states of a set of quantum systems are identical or not. This paper treats both error-free comparison, and comparison where errors in the result are allowed. Error-free comparison means that we aim to obtain definite answers, which are known to be correct, as often as possible. In general, we will have to accept also inconclusive results, giving no information. To obtain a definite answer that the states of the systems are not identical is always possible, whereas, in the situation considered here, a definite answer that they are identical will not be possible. The optimal universal error-free comparison strategy is a projection onto the totally symmetric and the different non-symmetric subspaces, invariant under permutations and unitary transformations. We also show how to construct optimal comparison strategies when allowing for some errors in the result, minimising either the error probability, or the average cost of making an error. We point out that it is possible to realise universal error-free comparison strategies using only linear elements and particle detectors, albeit with less than ideal efficiency. Also minimum-error and minimum-cost strategies may sometimes be realised in this way. This is of great significance for practical applications of quantum comparison.Comment: 13 pages, 2 figures.     A uvbyCaHbeta CCD Analysis of the Open Cluster Standard, NGC 752

    Precision uvbyCaHbeta photometry of the nearby old open cluster, NGC 752, is presented. The mosaic of CCD fields covers an area ~42' on a side with internal precision at the 0.005 to 0.010 mag level for the majority of stars down to V~15. The CCD photometry is tied to the standard system using an extensive set of published photoelectric observations adopted as secondary standards within the cluster. Multicolor indices are used to eliminate as nonmembers a large fraction of the low probability proper-motion members near the faint end of the main sequence, while identifying 24 potential dwarf members between V=15.0 and 16.5, eight of which have been noted before from Vilnius photometry. From 68 highly probable F dwarf members, we derive a reddening estimate of E(b-y)= 0.025 +/- 0.003 (E(B-V) = 0.034 +/- 0.004), where the error includes the internal photometric uncertainty and the systematic error arising from the choice of the standard (b-y), Hbeta relation. With reddening fixed, [Fe/H] is derived from the F dwarf members using both m_1 and hk, leading to [Fe/H] = -0.071 +/-0.014 (sem) and -0.017 +/- 0.008 (sem), respectively. Taking the internal precision and possible systematics in the standard relations into account, [Fe/H] for NGC 752 becomes -0.03 +/-0.02. With the reddening and metallicity defined, we use the Victoria-Regina isochrones on the Stromgren system and find an excellent match for (m-M) = 8.30 +/- 0.05 and an age of 1.45 +/- 0.05 Gyr at the appropriate metallicity.Comment: 37 pages, 11 figures, 4 tables.     Synthetic and Evolutionary Construction of a Chlorate-Reducing Shewanella oneidensis MR-1.

    UnlabelledDespite evidence for the prevalence of horizontal gene transfer of respiratory genes, little is known about how pathways functionally integrate within new hosts. One example of a mobile respiratory metabolism is bacterial chlorate reduction, which is frequently encoded on composite transposons. This implies that the essential components of the metabolism are encoded on these mobile elements. To test this, we heterologously expressed genes for chlorate reduction from Shewanella algae ACDC in the non-chlorate-reducing Shewanella oneidensis MR-1. The construct that ultimately endowed robust growth on chlorate included cld, a cytochrome c gene, clrABDC, and two genes of unknown function. Although strain MR-1 was unable to grow on chlorate after initial insertion of these genes into the chromosome, 11 derived strains capable of chlorate respiration were obtained through adaptive evolution. Genome resequencing indicated that all of the evolved chlorate-reducing strains replicated a large genomic region containing chlorate reduction genes. Contraction in copy number and loss of the ability to reduce chlorate were also observed, indicating that this phenomenon was extremely dynamic. Although most strains contained more than six copies of the replicated region, a single strain with less duplication also grew rapidly. This strain contained three additional mutations that we hypothesized compensated for the low copy number. We remade the mutations combinatorially in the unevolved strain and determined that a single nucleotide polymorphism (SNP) upstream of cld enabled growth on chlorate and was epistatic to a second base pair change in the NarP binding sequence between narQP and nrfA that enhanced growth.ImportanceThe ability of chlorate reduction composite transposons to form functional metabolisms after transfer to a new host is an important part of their propagation. To study this phenomenon, we engineered Shewanella oneidensis MR-1 into a chlorate reducer. We defined a set of genes sufficient to endow growth on chlorate from a plasmid, but found that chromosomal insertion of these genes was nonfunctional. Evolution of this inoperative strain into a chlorate reducer showed that tandem duplication was a dominant mechanism of activation. While copy number changes are a relatively rapid way of increasing gene dosage, replicating almost 1 megabase of extra DNA is costly. Mutations that alleviate the need for high copy number are expected to arise and eventually predominate, and we identified a single nucleotide polymorphism (SNP) that relieved the copy number requirement. This study uses both rational and evolutionary approaches to gain insight into the evolution of a fascinating respiratory metabolism

    Thermal kinetic inductance detectors for ground-based millimeter-wave cosmology

    We show measurements of thermal kinetic inductance detectors (TKID) intended for millimeter wave cosmology in the 200-300 GHz atmospheric window. The TKID is a type of bolometer which uses the kinetic inductance of a superconducting resonator to measure the temperature of the thermally isolated bolometer island. We measure bolometer thermal conductance, time constant and noise equivalent power. We also measure the quality factor of our resonators as the bath temperature varies to show they are limited by effects consistent with coupling to two level systems.Comment: 8 pages, 4 figures.
